Online Video Conference Registration
Thank you for your interest in the Ombudsman Office for State Managed Health Care Programs video conference series. The agenda for each quarterly meeting will be available online approximately one month prior to the meeting; materials will be available online one week prior. For a listing of future sessions, please see the calendar on the CountyLink web site. Registration is typically open four weeks prior to the meeting date.
Questions with an "*" are mandatory. When finished, a confirmation page will be shown for you to print. It includes a confirmation number that is required if you need to cancel your registration. You will be asked to select which site you wish to register for. Each site has a maximum number of seats. If your desired location is filled, it will not show in the list of available sites.
Meeting materials will be available online approximately one week prior to the session. It is the individual’s responsibility to print the materials for the session. Individuals with a disability who need a reasonable accommodation to participate in this event should click on the question that asks for "special needs." A staff person will contact you by e-mail for more details. Please note that it may take up to four weeks to arrange for an ASL interpreter.
